Position: Clinic Nurse (RN) - Family Medicine - $5,000 Sign On Bonus/$5,000 Loyalty Bonus

Our service area covers 14 counties, from the front range to the Kansas border, with locations throughout Colorado Springs, Fountain, Divide, Limon, and Strasburg. Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Compensation (Pay): $32.39 to $46.96 hourly based on experience. An additional $2.00 per hour if you hold a BSN.

Sign‑on and Loyalty Bonuses:

  • $5000 sign‑on bonus paid in installments: $1250 after 30 days, $1250 after 90 days, $1250 after 6 months, and $1250 after 9 months. Current employees are not eligible.
  • Loyalty bonus of $5000 upon 1 year of service and $1250 for every additional 3 months worked. Applies only to full‑time employees; prorated for part‑time.

Job Summary:

The Care Team Nurse functions as a liaison between the patient and members of the Care Team in a joint effort to optimize the patient’s health status and quality of life. The Care Team Nurse will perform routine and emergent nursing functions pertinent to efficient, ambulatory patient care, while following the guidelines set forth by the Colorado Nurse Practice Act.

Essential Duties and Responsibilities:
  • Assesses and triages for the appropriate level of care based on comprehensive risk assessment.
  • Assesses patient’s readiness for change, physical and psychosocial needs.
  • Identifies and links patients to available resources, social support systems, and family support.
  • Assesses high‑risk and rising‑risk patients consistent with current policies for Care Management.
  • Facilitates access to quality and cost‑effective care.
  • Collaborates with and maintains healthy working relationships with community partners.
  • Provides education to patients, families, caregivers.
  • Encourages engagement and self‑management of patients regarding their health.
  • Provides outreach to individuals and the community.
  • Provides in‑reach to patients who are presently inactive in their healthcare.
  • Demonstrates ability to utilize technology including Electronic Health Records, state registries, Health Information Exchange.
  • Maintains documentation of all interactions accurately and completely, implementing appropriate protocols for patients’ charts and billing practices.
  • Maintains healthy working relationships with team members and staff from all levels of the organization.
  • Supports the clinical training department and assures Medical Assistants are competent regarding clinical skills.
  • Assists providers with inbox tasks including medication refills per standing order SOP and completing pertinent paperwork for patients outside needs.
  • Triages and appropriately delegates follow‑up needs for patients regarding labs and any outside testing.
  • Advocates for patients.
  • Provides education as needed on disease processes and medication regimen.
  • Works with care teams to assure schedules reviewed and patients’ charts prepared for visits.
  • Understands immunization schedule and supports clinic staff and patients.
  • Operates corporate or personal vehicles, if applicable, safely and professionally at all times; travels to any/all of Peak Vista’s facilities as needed.
