We pursue the best possible outcome as defined by the client, family members, and care team. NTSOC is looking for a compassionate LPN to provide high quality nursing care for our pediatric and adult clients to improve their quality of life at home.
Job Type:- Full time and Part time;
Compensation: $24.00-$33.00 per hour dependent upon experience.
- Health, Vision and Dental Insurance, HSA, PTO/Sick Time, 403b with Company Match, Voluntary Life and AD&D, Short/Long Term Disability, Critical Illness Insurance.
- Provide one on one care to a single client in their home setting and while in the community.
- Implements the client’s physician ordered plan of care and nursing interventions while adhering to agency policies.
- Perform assessments to monitor the health status of the client.
- Notify the physician and Field Nurse Case Manager of any changes in the client’s condition.
- Coordinate care with the Field Nurse Case Manager and other members of the client’s multidisciplinary care team.
- Administer medications and treatments as ordered.
- Document performed interventions and treatments.
- Assists clients with activities of daily living such as bathing, toileting, dressing, and grooming.
- Collaborate with the care team on implementation and updates to the client’s plan of care.
- Monitor client progress toward home health care goals.
- Educate the client and family regarding the plan of care and nursing care needs.
- To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

- Graduate of an Accredited Nursing Program required.
- 1-2 years home health care experience preferred and 1 year of nursing experience in a health care setting preferred.
- Strong communication skills.
- Strong computer skills.
- Colorado LPN license in good standing with the Department of Regulatory Affairs.
- BLS required.
- Up to date car insurance and driver license will be required.
