Role Description

This full‑time, on‑site Registered Nurse role is based in Colorado Springs, CO. The Registered Nurse will provide direct home health nursing care to patients with complex medical and behavioral health needs, including assessment, care planning, treatment, and patient education focused on holistic well‑being. Daily responsibilities include coordinating with interdisciplinary team members and external providers, documenting care in the EMR, monitoring patient progress, and adjusting care plans as needed.

The nurse will teach patients and caregivers disease management and medication adherence, promote safety and preventive care, and respond to changes in condition in a timely and professional manner. The role also involves participating in case conferences, following regulatory and agency standards, and contributing to quality improvement initiatives.

Qualifications
  • Current, active Registered Nurse (RN) license in the state of Colorado; graduation from an accredited nursing program.
  • Clinical skills in patient assessment, care planning, wound care, medication administration, and symptom management.
  • Experience or strong interest in home health, behavioral health, or caring for patients with multiple comorbidities.
  • Proficiency in electronic medical records (EMR), cloud‑based documentation systems, and basic computer skills.
  • Strong communication, education, and counseling skills for working with patients, families, and interdisciplinary teams.
  • Ability to work independently in the field, manage time effectively, and prioritize multiple patient needs.
  • Commitment to data‑driven, evidence‑based practice and continuous quality improvement.
  • Valid driver’s license, reliable transportation for home visits, and ability to meet any required background and health screenings.
