• Provide skilled nursing care and treatments as ordered by the physician
• Monitor patient conditions and report changes to the RN case manager and physician
• Administer medications and perform wound care and other nursing procedures
• Educate patients and families on treatment plans, medications, and disease management
• Accurately document patient care and progress
• Collaborate with the interdisciplinary care team to support patient goals
