Qualifications & Skills- Current, unrestricted APRN or PA license in Colorado
- Prescribing authority and DEA registration or ability to obtain
- Experience in family, geriatrics, or adult care
- Flexibility to work across various care settings
- Reliable transportation and valid driver’s license
- Proficiency with medical coding, value-based care, and EMR tools is preferred.
- Strong communication, documentation, and patient education skills.
- Conduct wellness visits and assessments
