RN, Nursing Home, RN Nurse

Skyline Ridge Nursing & Rehabilitation Center in Canon City, CO is seeking a compassionate RN to deliver person-centered care in our skilled nursing and rehab community. The RN will provide clinical assessments, administer medications and treatments, coordinate care plans, and support residents' independence and dignity. Responsibilities include supervising CNAs, collaborating with an interdisciplinary team, maintaining accurate documentation, and promoting safety and infection control.

Responsibilities

• Provide direct nursing care to residents, including assessments, treatments, and medication administration per provider orders
• Monitor resident conditions, respond to changes in status, and communicate promptly with providers and families
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
• Supervise and support CNAs and other care staff to ensure safe, high-quality, person-centered care
• Document assessments, interventions, and outcomes accurately and timely in the medical record
• Participate in care conferences, shift reports, and quality improvement initiatives
• Educate residents and families on care plans, treatments, and health management
• Follow infection control, safety, and regulatory guidelines to maintain a safe care environment
