Licensed Practical Nurse

Columbine Manor Care Center in Salida, CO is seeking a Licensed Practical Nurse to deliver compassionate, resident-centered care in our skilled nursing and rehabilitation facility. The LPN will provide direct bedside care, administer medications and treatments, monitor vital signs, and support individualized care plans for long-term and short-stay residents. You'll collaborate with RNs, CNAs, therapists, physicians, and families to promote dignity, safety, and quality of life.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments according to provider orders and facility policy
• Monitor and document vital signs, resident status, and responses to care
• Implement and support individualized care plans in collaboration with the care team
• Communicate changes in resident condition to RNs, providers, and families
• Supervise and support CNAs to ensure high-quality, resident-centered care
• Maintain accurate, timely electronic and written documentation
• Follow infection control, safety, and regulatory guidelines
• Participate in interdisciplinary team meetings and care conferences
• Educate residents and families on care, treatments, and health goals
