Requirements:
Current, unrestricted state Registered Nurse license;
Associate or bachelor's degree in nursing or related field;
Ability to analyze complex medical information and make rational decisions;
Case Management certification.

• Perform PRN/focused and routine assessments of high risk members.
• Collaborate with primary care providers to develop and implement individualized care plans.
• Address, identify, and reassess appropriate levels of care, including non-medical support to ensure treatment compliance.
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
• Keep detailed records of clinical, functional, and fiscal outcomes during the management process.
