Wound Care - Registered Nurse (RN)About the Position
We are seeking a dedicated and compassionate
Wound Care Registered Nurse (RN) to join our nursing team. The Wound Care RN will oversee wound management and skin integrity throughout our community, providing clinical expertise in wound assessment, treatment, prevention, documentation, and staff education.
This position is ideal for an RN who enjoys combining hands-on clinical care with education, leadership, quality improvement, and resident-centered care.
What You’ll Do- Complete comprehensive skin and wound assessments, including measurements, staging, photographs, and documentation.
- Develop and monitor individualized wound care treatment plans and perform treatments within scope of practice.
- Identify residents at risk for pressure injuries and lead prevention initiatives.
- Monitor wounds for infection, changes in condition, and treatment effectiveness.
- Participate in wound rounds and collaborate with nursing, physicians, therapy, dietary, and other interdisciplinary team members.
- Educate and mentor nursing staff on wound care, skin integrity, prevention, treatment, and documentation.
- Provide wound care education to residents and families.
- Maintain accurate documentation in the electronic medical record.
- Monitor wound care quality indicators and participate in QAPI initiatives.
- Support compliance with CMS, Colorado regulations, infection prevention standards, and community policies.
- Assist with surveys, audits, and regulatory inspections.
- Participate in rotating nurse on-call responsibilities.
- Current CPR certification.
- Minimum1 year of nursing experience in Skilled Nursing, Rehabilitation, Long-Term Care, Acute Care, or a related healthcare setting.
- Knowledge of wound assessment, treatment, pressure injury prevention, and skin integrity management.
- Wound Care Certification preferred; ability to obtain certification within 90 days of hire.
- Point Click Care or other EHR experience preferred.
- Knowledge of CMS and long-term care regulations preferred.
- Strong clinical assessment, critical thinking, organization, and communication skills.
- Ability to educate and mentor nursing staff.
- Proficiency with Microsoft Word, Outlook, and Excel.

Schedule:
Monday through Friday and rotating Engaged Standby-The team member must be able to respond to phone calls/texts during normal off-duty hours and must respond by phone/video call and frequently be required to come in to work on-site within a specified period of time after receiving the call.
