Responsible to coordinate the Family Self‑Sufficiency (FSS) and Resident Services programs as required by the Colorado Springs Housing Authority’s Annual Plan and the U.S. Department of Housing and Urban Development’s (HUD) rules and regulations where applicable. The Supportive Services Coordinator will recruit participants from current Housing Choice Voucher (HCV) participants and CSHA tenants. Assumes responsibility for case management including development of contacts with service providers and agencies for referrals and the ongoing case management responsibility.
ESSENTIALJOB FUNCTIONS
- Recruits FSS participants from current families and/or individuals on the Housing Authority’s Housing Choice Voucher Program.
- Recruits Resident Services participants from current families and/or individuals residing in the Housing Authority’s properties.
- Select and execute contracts with these families in accordance with the Annual Plan established by the PHA and appropriate HUD regulations.
- Assists the participants with goal setting, advocacy, counseling, emotional support, crisis intervention and problem solving once participants have executed contracts.
- In partnership with other Community organizations, assist program participants and eligible families with the understanding, navigation, and access of community services.
- Devises and maintains individual participant case files and computerized monitoring system to track client participation. Case files must contain copies of the Contract of Participation, monthly and quarterly progress reports, client information, copies of escrow account statements, and other information as applicable.
- Ensure that the services and supports included in the participants’ Contracts of Participation are provided on a regular, ongoing, and satisfactory basis; participants are fulfilling their responsibilities under the contract; and FSS escrow accounts are established and properly maintained for eligible families.
- Ensure that an Individual Training and Services Plan (ITSP) is prepared for head of household and each adult member of the household who elects to participate in the program.
- Provides quarterly progress reports. Reports will include the number of participants employed, attending school, progress with achieving goals, and related information.
- Establishes a PCC (Program Coordinating Committee) - consisting of members of the community, maintains an equal balance of members (as required by HUD regulations), and schedules quarterly meetings with the PCC.
- Builds partnerships with employers and service providers in the community, and work with the PCC and with such local service providers to ensure that program participants are linked to the supportive services they need to achieve self‑sufficiency, including services for participants with limited English proficiency.
- May also perform job development functions (e.g. outreach to potential employers) for the FSS program.
- Deliver supportive services to CSHA families by conducting ongoing outreach and engagement activities.
- Create, maintain, and update the FSS Action Plan and FSS Program Policies with current HUD regulations and policies set by CSHA.
- Acts as liaison between the PCC, FSS participants, and CSHA Management Staff.
- Establishes and maintains cooperative relationships with area human service agencies and informs and refers clients appropriately.
- Coordinates with community agencies to provide monthly client educational presentations.
