Lead Case Manager – Commons

The Lead Case Manager provides case management at the Commons Permanent Supportive Housing (PSH) community for Veterans, families, and individuals. The Lead Case Manager acts as a lead to provide support to the team of case managers and housing assistants, and will also carry a caseload of clients. This position reports to the Director of Supportive Housing.

• Responsible for supporting the Director of Supportive Housing with the oversight The Commons, a Permanent Supportive Housing (PSH) community for Veterans, families, and individuals.
• Effectively support other staff to include supporting core values, addressing zero tolerance behaviors, and maintaining active communication with employees.
• Work in conjunction with the property management company to ensure building operations are followed in accordance with policies and procedures.
• Review and assist with guidance various proposals and contracts.
• Assist in coordinating, managing, and monitoring group work, ensuring that life skills groups are provided, documented, and billed regularly.
• Assist supervisor with training new and incumbent staff, evaluate training needs, implement policies and procedures, and provide feedback to the supervisor.

• Serve as Case Manager for client/residents while carrying a caseload: work with each client to set goals for healthy individual growth with an emphasis on maintaining housing; monitor progress in attaining goals; monitor adherence to all Homeward Pikes Peak policies and procedures specific to The Commons.
• Provide transportation for clients to appointments when necessary.
• Coordinate with other community service agencies to access services for clients as needed.
• Maintain accurate records, which include client charts as well as internal and external reports.
• Develop Individualized Services Plans for clients.
• Assist in maintaining systems to ensure client records and databases are up to date, ensure that client interactions are timely and appropriately documented in the HMIS system, and adhere to appropriate confidentiality standards relative to data. Ensure Reliatrax is utilized regularly to support the billing of supportive services.
• Assist with maintaining updated policies and procedures, training manuals for assigned programs, and assist with overseeing the creation and implementation of yearly training for all staff.
• May attend appointments with clients.
• Support clients’ vocational choices and assist them in choosing a job that matches their strengths, overcoming job-related anxiety by reviewing job applications, and providing interview tips.
• Assist clients in building social skills in the community that will enhance job acquisition and tenure.
• Identify any problems, challenges, or obstacles in coordination of services, identify potential solutions, and resolve such problems, challenges, or obstacles in a timely manner.
• Document intervention activities in an accurate and timely manner and seek consultation from supervisor or other staff as necessary.
