Cancellation Policy:
Customer may cancel a Provider's assignment without cause at any time upon providing notice thereof. If Customer cancels a Provider's assignment less than fourteen (14) day prior to the scheduled commencement of such assignment by a fully credentialed Provider, Agency may invoice Customer for twelve (12) hours at the proposed assignment weekday bill rate, and to the extent substantiated by written documentation, for actual fees and charges that may result from cancellation of an Assignment, including but not limited to lost rents up to a maximum of one month, security deposits and airfare ("Costs").

After a Provider has begun working on an assignment, Customer may terminate the assignment without any penalty if a thirty (30) day advance notice is provided to Agency. If less than a thirty (30) days advance notice is given by Customer, then Customer will pay for thirty-six hours at the Provider's current assignment weekday bill rate. Agency Cancellation. If Agency cancels a Provider's travel assignment less than fourteen (14) days before the start of that assignment, Agency will provide Customer with a twelve (12) hour credit on the next invoice.

If, after a travel assignment has commenced, Agency terminates an assignment for a Travel Provider other than for a material breach of this Agreement by Customer without giving Customer at least thirty (30) days prior written notice, then Agency shall pay Customer an amount equal to three (3) 12-hour shifts. Minimum Hours. Unless otherwise designated in the applicable SOW, for Travel Providers confirmed for eight (8) or ten (10) or any combination of eight (8), ten (10) and twelve (12) hour shifts, Customer will offer such Providers a minimum of forty (40) scheduled hours weekly.

For Travel Providers confirmed for twelve (12) hour shifts, Customer will offer such Providers a minimum of thirty-six (36) scheduled hours weekly. These guaranteed hours do not include any "on-call" time worked by a Provider. Notwithstanding the foregoing, in case of low census or other call-offs and cancellations, Agency may bill Customer and Customer agrees to pay the short-rate per hour as specified in the SOW and corresponding Assignment Agreement.

Traveler Providers engaged in long term assignments will make reasonable efforts to re-schedule missed shifts due to sick days within the term of their assignment.
