Schedule:
June Dates: 23, 24, 26, 30
July Dates: 1, 9, 15, 16, 20-24, 28, 29, 31
Monday - Thursday 8:30am - 4:00pm
Fridays 7:30am - 2:00pm
Setting:
Clinic
Procedures/Cases:
All child and adolescent patients. Procedures needed: exams, stainless steel crowns, simple extractions, composite fillings, pulpotomies.
Credentialing Timeframe: ~1-2 days
Requirements:
Active state license, Drug Enforcement Administration (DEA), Basic Life Support (BLS), Cardiopulmonary Resuscitation (CPR), active Medicaid and use of Curve Hero Electronic Medical Record (EMR)
