SWAP (School to Work Alliance Program) Specialist – Lake County/Leadville

Location: Lake County, Colorado (Leadville)
OverviewSWAP assists young adults ages 15-24 with mild to moderate needs in employment to become competitively employed and to achieve successful community outcomes. The SWAP Specialist provides these youth with pre‑employment and employment services both in school and in the community, working through Mountain BOCES in partnership with the Division of Vocational Rehabilitation (DVR), local school districts, Colorado Department of Education (CDE), and area businesses.
Responsibilities- Referral and Intake – Collaborate to schedule and complete intake for applicants, including obtaining school records, completing DVR Release of Information, and supporting eligibility determination.
- Eligibility, Comprehensive Assessment & IPE Development – Develop and annually update Individual Plans for Employment (IPE) and coordinate with DVR and education partners.
- Service Provision – Deliver pre‑employment transition services such as job exploration, workplace readiness, self‑advocacy, work‑based learning, and vocational guidance.
- Case Closure & Follow‑Along Services – Ensure participants remain employed for one year after DVR case closure through periodic contacts and support coordination.
- Programmatic Documentation – Submit monthly progress reports, maintain required filing, and provide documentation to education partners.
- Partnerships – Develop and maintain relations with local education staff, community providers, employers, and DVR to promote vocational rehabilitation opportunities.
- Associate’s degree in a related field or equivalent in years of experience.
- Minimum of two (2) years of experience in job development, job coaching, assessment, or job‑seeking skills training.
- Experience working with diverse populations of young adults with disabilities.
- Proficiency in Microsoft Word, Excel, PowerPoint, Google Docs, Google Sheets, Google Slides, and virtual meeting platforms.
- Criminal background check; ability to travel among school and community locations.
- Heavy lifting; some travel required.
- Office‑based and field‑work across multiple schools and community sites.
Contract Days: 200 (FTE=1.0), Beginning 7/1/2026 for FY26‑27
Salary: Hiring Range $35,000 – $60,000; placement on hiring schedule depends on education & experience.
