About the opportunity:

The Behavioral Health Case Manager is responsible for comprehensive case management services including comprehensive discharge planning across the Vail Health Behavioral Health service line and the behavioral health service lines in the patient’s community. The goal of the Case Manager position is to reduce barriers to care, enhance coordination of care for patient external and internal to behavioral health systems, improve behavioral health outcomes through supportive case management, access to services to address social determinants of health and behavioral health, early intervention, risk reduction, patient education and empowerment.

The Behavioral Health Case Manager will work within the context of a patient-centered, strength based, recovery-oriented, wraparound approach, in partnership with behavioral health providers, agencies, and medical team members to promote timely access to needed care and services, continuity of care, client empowerment, and the enhancement of patient and family wellbeing

What you will do:

  • Under the supervision of the Director of Case Management, the Behavioral Health Case Manager provides comprehensive case management services across behavioral health service lines to ensure a consistent and coordinated delivery of services for high-risk, high-acuity patients.
  • Manages a comprehensive caseload that is based on community and operational need, patient population need, and services delivered across the service community and across all Behavioral Health service locations. It is expected that this position maintains a caseload of 32 billable hours per week, dependent on patient volume.
  • Maintains excellence in clinical documentation in alignment with local, internal, state, federal, and regulatory standards and compliance.
  • Engages in comprehensive service plans and client assessments that address clinical need, level of care and associated factors that may impact care (i.e. SDoH).
  • Completes comprehensive discharge planning for patients discharging from higher levels of care.
  • Supports care coordination with behavioral health and medical providers to improve access and continuity of care. This level of engagement may include home visits, assistance in attending appointments, or other activities that support continuity of care.
  • Supports the development of case management service plan, setting of goals, and coordinating/attending care coordination meetings with providers and community partners as indicated to support the needs of the client.
  • Engages in comprehensive facilitation of referrals, resources, and the coordination with community resources associated with total health outcomes.
  • Supports patient, family, and community-level education, prevention, and skill development to increase client patient and family outcomes.
  • Monitors patient outcomes, experience, and feedback and collaborates with team members to develop strategies to promote positive experiences with a commitment of improving quality of care delivery.
  • Analyzes data and other sources of evidence to inform standards, practices, and patient care models. Ensures clinical policies and procedures are reviewed and updated in accordance with evidence-based practice. Prepares reports, maintains records, and compiles data and statistics as directed/required.
  • Maintains the security and confidentiality of data/information used to perform daily duties adhering to hospital policy, laws, and regulations as appropriate.
  • Role models the principles of a Just Culture and Organizational Values.
  • Performs other duties as assigned.
