Job Summary
: A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis at their place of residence, following physician orders and a plan of care under the direction and supervision of the Clinical Manager/Nursing Supervisor.

- Graduate from a PT curriculum approved by the American Physical Therapy Association or equivalent.
- Currently licensed in the state(s) of practice.
- Two (2) years of experience preferred.
- Acceptance of the Agency’s philosophy and goals.
- Ability to exercise independent judgment and initiative.
Reports to the Clinical Manager/Nursing Supervisor/Therapy Supervisor.
Responsibilities- Assist the physician or intermediate care provider in evaluating functional level and developing the plan of care.
- Prepare clinical and progress notes.
- Advise and consult with families and agency personnel.
- Participate in and present at in‑service programs.
- Adhere to established policies and procedures.
- Collaborate with all healthcare personnel in patient care planning.
- Direct and supervise assistant personnel (PTA, HHA) as required.
- Complete initial evaluations, notify physicians of patient needs, and submit plans for approval.
- Perform skilled PT procedures as ordered by the physician.
- Consult physicians regarding treatment changes.
- Write reports to physicians on patient progress.
- Instruct patients and families in home programs and activities of daily living.
- Attend all staff meetings and patient care conferences.
- Supervise PTA every 30 days and HHA as requested.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions, discharges, and significant condition changes within defined time frames.
- Use ICD‑10 diagnoses and codes appropriately.
- Complete discharge summaries and recertification reports every 60 days when continuing treatment.
- Protect patient confidentiality and safeguard related information.
- Prepare and submit patient evaluations and notes promptly.
- Report incidents or complaints to Clinical Manager or Nursing Supervisor immediately.
- Participate in peer review and QAPI activities.
- Engage in utilization review of medical records.
- Maintain attendance and appearance per Agency standards.
- Demonstrate effective time management, sound judgment, critical thinking, and decision making.
- Maintain a clean and neat work environment.
- Perform other duties as assigned.
Work indoors at the agency office and at patient homes; travel to and from patient homes as required.
Job RelationshipSupervised by:
Clinical Manager/Nursing Supervisor/Therapy Supervisor.
Workers supervised:
Physical Therapy Assistant and Home Health Aide.
Compensation negotiable based on experience. Base hourly rate for meetings and visits paid per visit.
