Senior Community Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.
Pay: $18.50 - $22.50 per hour, based on previous work experience.
Schedules: Fulltime, 8–10-hour shifts, days and times will vary, occasional weekend shifts.

The Personal Care Provider is responsible for providing and assisting participants with activities of daily living and supportive care necessary to the preservation of assisting participant’s in living independently in the community. Assists in providing a safe environment for the participants of Senior Comm Unity Care. Functions as a member of Interdisciplinary Team.
QUALIFICATIONS:
- High School diploma or equivalent required
- A minimum of one year’s experience in working with the frail or elderly population required.
- Active CPR/First Aid Certification
- Must have a valid driver’s license, proof of insurance and have means of transportation
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact
Skills and Knowledge:
- Basic knowledge of how to support frail or elderly in Activities of Daily Living (ADL).
- Knowledge of how to use and maintain in-home equipment (adaptive).
- Skilled in establishing and maintaining effective working relationships with participants, co- workers, medical staff, and the public.
- Skilled in identifying and recommending problem resolution.
- Ability to work independently with minimum supervision.
- Ability to communicate clearly and effectively.
- Ability to work with frail and demented population.
- Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
ESSENTIAL FUNCTIONS:
- Assist participants with non-skilled activities of daily living (ADL’s) in the home and in the Day Center including but not limited to bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting as assigned or needed.
- Assists participants with non-skilled restorative and supportive care in the home and in the Day Center including ambulating, positioning, transferring, skin care, personal care, etc. as assigned or needed.
- Assists participants with Independent Activities of Daily Living (IADL’s) including, but not limited to, grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care as assigned.
- Assists with participant activities in the Day Center, including meal service and clean-up.
- Responsible for daily cleaning of the Day Center, including maintenance of cleanliness throughout the day and following all infection control measures related to Day Center cleaning.
- Completes appropriate documentation of services provided following Senior Comm Unity Care policy.
- Implements Personal Care Services plan of care, contacts supervisor with any clarifications.
- Reports changes in participant’s condition and needs to the Personal Care Supervisor (or designee).
- Acts within the scope of his or her authority to practice.
- Participates in and supports Quality Improvement Initiatives.
- Follows all Senior Comm Unity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
- Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
