About Senior Comm Unity Care of Colorado (PACE)
Senior Comm Unity Care of Colorado is part of Volunteers of America National Services and the first PACE provider in Western Colorado. Through the Program of All-Inclusive Care for the Elderly (PACE), we support adults age 55+ in remaining independent and living safely in their homes and communities.

Position OverviewWe are seeking a Registered Dietitian to provide comprehensive nutritional assessment, intervention, and education to PACE participants. This role works closely with an interdisciplinary care team to address both medical and social factors impacting nutrition and overall health.
Care is delivered at the PACE center and in participant homes, serving a census of approximately 170 participants.
Key Responsibilities Clinical Assessment & Intervention- Conduct nutritional screenings and comprehensive assessments
- Provide Medical Nutrition Therapy (MNT) and individualized nutrition education
- Identify root causes affecting nutrition (financial barriers, access to food, mobility, etc.)
- Monitor weights, intake, hydration, swallowing concerns, and drug–nutrient interactions
- Document assessments and interventions in the EHR
- Communicate nutritional findings to Physicians, RNs, and IDT members
- Participate in care planning and IDT meetings
- Coordinate nutrition-related interventions to support holistic care
- Oversee meal service and kitchen operations for quality and regulatory compliance
- Collaborate with contracted meal vendors to ensure menu accuracy and satisfaction
- Manage dietary inventory, supplements, and therapeutic diets
- Supervise and support the Dietary Aide
- Provide nutrition education to healthcare staff
- Participate in quality improvement initiatives
- Degree from an accredited college in Food & Nutrition, Dietetics, or related field
- Registered Dietitian (RD/RDN) certification
- Valid driver’s license and reliable transportation
- Experience working with an elderly or frail population
- Strong communication and organizational skills
- Ability to work independently and within a team setting
