Senior Comm Unity Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.

Pay

$22.50 - $25 per hour based on experience

Schedule

Monday - Friday 8a-5pm. Travel in the community to and from participants' homes may be required.

The Social Work Case Manager will support the Social Work Department in the care and treatment of participants and to maintain the highest physical, mental and psycho-social well-being of each participant. To serve as a liaison to community agencies on behalf of Senior Comm Unity Care PACE. To support participants with access to community resources for services provided outside of the PACE program, such as housing, food assistance, utility assistance, legal aide, etc.

QUALIFICATIONS
  • High school diploma or equivalent education required; bachelor’s degree in social work or related degree in social sciences preferred.
  • Minimum of one year experience with older adults.
  • Experience and knowledge of community resources available to older adults in respective community.
  • Knowledge of long-term health care and public health settings and familiarity with Federal and State regulations and requirements for long-term care.
  • Ability to read, speak and write fluently in English. Bilingual preferred.
  • Excellent interpersonal communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations.
  • Must demonstrate ability to work with a diverse team and flourish in a flexible work environment.
  • Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions and decimals; ability to comprehend and execute simple statistical functions to create standard graphs and tables.
  • Ability and willingness to work in various locations, for planned activities or special events, family meetings and conferences.
  • Negotiation skills and ability to carry on several levels of reasoning at once during problem solving sessions; ability to think quickly and to write complex written arguments.
  • Ability to work with participants of varying levels of responsiveness.
  • Familiarity with word processing, data processing and computer entry skills to match existing clinical management software at facility.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in‑servicing and documentation.
  • Ability to perform several job functions at once.
ESSENTIAL FUNCTIONS
  • Receives referrals from Social Workers or other PACE Interdisciplinary Team (IDT) members when participant resource needs are identified.
  • Meets with participants, families, IDT and other community members to coordinate referrals for participants to access resources in the community.
  • Assists participants with completing paperwork and gathering necessary documents to complete resource applications.
  • Implements case management or other Social Work interventions as assessed and delegated by Master’s Level Social Worker (MSW). Works in tandem with MSW to ensure interventions are provided within scope of practice and to meet the psychosocial needs of the PACE participants.
  • May assist the Center Director or Social Worker with coordinating invitation of Participants, Families and/or IDT members for Participant Care Conferences.
