Licensed Clinical Social Worker; Craig Clinic

Position: Licensed Clinical Social Worker (Craig Clinic)
Location: Meeker

Reports To : CEO

FLSA Classification :
Full-Time, Exempt, $31.42- $45.75

Essential Functions :

* Provide therapeutic services to patients and their families.

* Work in collaboration with multidisciplinary team members including external partners.

* Completes required documentation and paperwork including assessments, treatment planning and implementation in timely manner.

* Develop and maintain social services, draft supporting policies, procedures and guidelines for the program. Develop and maintain a knowledge of referral resources throughout the service area.

* Act as a resource person for other members of the healthcare team, provide in-service education and consultation of social and emotional factors related to health problems.

* Maintain knowledge of the field including Medicare, Medicaid and other third-party reimbursement sources.

Required Skills/Abilities:

* Working knowledge of clinic policies and procedures

* Knowledge and experience working with co-occurring populations, chronic health, mental health, or substance abuse.

* Strong clerical/administrative skills including computer skills.

* Ability to make routine decisions and determine proper action based on experience with the limits of policy/procedures.

* Demonstrated oral/written skills in English
