Occupational Therapist (OT)
Location: River Valley Rehabilitation & Healthcare Center – Del Norte, CO
Position: Full‑Time

- Current Occupational Therapist (OT) license in Colorado or eligibility for Colorado licensure
- Graduate of an accredited Occupational Therapy program
- Skilled Nursing Facility (SNF) or Long‑Term Care (LTC) experience preferred, but new graduates are encouraged to apply
- Strong communication, clinical, and documentation skills
- Passion for delivering compassionate, resident‑centered care
- Evaluate residents and develop individualized occupational therapy treatment plans
- Provide skilled occupational therapy services to residents in a skilled nursing and long‑term care setting
- Accurately document evaluations, treatments, progress notes, and discharge summaries
- Collaborate with physicians, nursing staff, and the interdisciplinary care team to achieve the best resident outcomes
- Educate residents, families, and caregivers on therapy goals and adaptive techniques
- Promote resident independence, safety, and quality of life through evidence‑based therapy
- Follow all state and federal regulations
- Adhere to facility policies and procedures
- Perform other duties as assigned
