Help Shape the Future of Rural Healthcare

Pagosa Springs Medical Center (PSMC) is seeking a motivated and collaborative Quality & Transformation Coordinator to support organizational improvement initiatives that enhance patient care, operational excellence, workforce development, and rural healthcare sustainability.

This part-time position serves as a key facilitator of quality improvement projects, strategic initiatives, grant‑funded programs, and organizational transformation efforts across multiple departments and service lines. Working closely with leaders, providers, staff, community partners, and external stakeholders, the Coordinator helps turn ideas into action by supporting project planning, implementation, monitoring, reporting, and continuous improvement activities.

This role is ideal for an experienced healthcare professional, quality specialist, project coordinator, nurse, healthcare administrator, or public health professional seeking a flexible schedule while continuing to make a meaningful impact on healthcare delivery.

Schedule
  • Part‑Time | 20 hours per week (0.5 FTE)
  • Flexible scheduling available based on organizational needs
  • On‑site presence preferred; hybrid work arrangements may be considered
  • Occasional attendance at meetings, trainings, and organizational events may be required
Primary Responsibilities Quality Improvement & Performance Excellence
  • Facilitate Quality Assurance and Performance Improvement (QAPI) initiatives across the organization
  • Support departments in identifying opportunities for improvement and developing action plans
  • Monitor performance metrics, outcomes, and project progress
  • Assist with implementation of evidence‑based improvement strategies
  • Support regulatory readiness and organizational quality goals
Project & Transformation Coordination
  • Coordinate strategic initiatives, transformation projects, and organizational improvement efforts
  • Develop project plans, timelines, meeting agendas, and progress reports
  • Track project milestones, deliverables, risks, and outcomes
  • Facilitate communication among departments and stakeholders
  • Support implementation and sustainability of grant‑funded programs and special initiatives
Data Analysis & Reporting
  • Collect, organize, analyze, and report performance and project data
  • Develop dashboards, presentations, and reports for leadership and committees
  • Monitor key performance indicators and assist with outcome measurement
  • Support data‑driven decision making throughout the organization
Collaboration & Stakeholder Engagement
  • Build strong relationships with leaders, providers, staff, and community partners
  • Facilitate meetings and multidisciplinary work groups
  • Support change management and implementation efforts
  • Assist with strategic planning and organizational development initiatives
Organizational Support
  • Participate in quality, compliance, and performance improvement committees
  • Support accreditation, regulatory, and grant reporting requirements
  • Assist with special projects as assigned
  • Promote a culture of continuous improvement and innovation
Qualifications Education
  • Bachelor's degree in Healthcare Administration, Public Health, Nursing, Business Administration, Organizational Leadership, or related field preferred
  • Equivalent combinations of education and experience will be considered
Experience
  • Three (3) years of healthcare experience preferred
  • Experience in quality improvement, project management, healthcare operations, performance improvement, compliance, or practice transformation preferred
  • Critical Access Hospital, Rural Health Clinic, or rural healthcare experience preferred
Preferred Knowledge & Skills
  • Knowledge of healthcare quality improvement methodologies and patient safety principles
