OverviewThe Public Health Caseworker I (Journey Level) works with a multidisciplinary public health team to assess client needs, provide psychosocial support, and facilitate referrals to basic needs and community resources. The role serves adults, children, youth, and families across community and clinic settings, while collaborating with other public service agencies and community partners. All employees participate in public health emergency response activities in accordance with public health standards and practices.
Responsibilities- Client Relations (70%) – Establish and maintain therapeutic client relationships in the community, home, or clinic. Coordinate with care team members to assess, plan, implement, and evaluate client care plans; modify treatment plans as needed. Teach clients healthy behaviors and best practices for family health. Support clients in setting realistic goals and measurable outcomes, conduct home visits, assess physical, emotional, social, and environmental needs, and evaluate progress toward targeted goals.
Provide education, resources, and referrals, identify gaps in community systems, and collaborate with community resources to link families to appropriate services. Participate in program evaluation and team meetings, and assist with outreach to meet programmatic deliverables. - Administrative (20%) – Maintain program information and policies, complete continuing education and position‑specific training, keep current on regulations, enter data into multiple computer programs, apply new computer skills, and ensure compliance with federal, state, and local regulations and Weld County policies. Maintain sensitive and confidential records, follow HIPAA and other confidentiality guidelines, and record proper documentation of client care and progress.
- Other Duties (10%) – Travel to all Health Department locations, maintain professional conduct and adhere to the Public Health Code of Ethics, provide exceptional customer service, coordinate community outreach, and represent the department to promote a positive public health image. Work scheduled hours and be available outside regular hours for emergency response and community events.
- Education
:
Bachelor’s Degree in Human Behavioral Science or related field. - Experience
:
One (1) year of professional work in maternal and public health, family practice, child health, pediatrics, case management, or a closely related field in a public health setting. - Licenses & Certifications
:
Valid Colorado driver’s license and use of a personal, insured vehicle; criminal background check; pre‑employment drug and substance screening. Must be reachable during emergencies. - Other

Work outside normal hours as needed, especially during emergencies. Position is non‑exempt from minimum wage and overtime requirements. Equal Pay for Equal Work Act requires pay-range disclosure.
