Adaptive Home Health is seeking a Physical Therapist (PT) for field-based home health assignments in our Texas-led network. You will own the caseload as a case manager, delivering comprehensive in-home PT visits and building a clear plan of care focused on gait/balance, transfers, strengthening, and home safety.
You’ll coordinate with the care team, document in HCHB, and supervise PTAs where appropriate. A Texas PT license and daily travel across the assigned territory are required.
