Location: Telluride

Responsibilities

  • Provide outpatient therapy to children, adolescents, adults, and families
  • Deliver services in homes, schools, community settings, and office locations
  • Complete biopsychosocial assessments, treatment plans, and clinical documentation
  • Provide individual, family, and collateral therapy sessions
  • Utilize evidence-based practices appropriate to client needs
  • Coordinate care with schools, primary care providers, child welfare agencies, probation, and other community partners
  • Participate in multidisciplinary treatment planning and case consultation
  • Maintain compliance with agency, payer, and regulatory documentation requirements
  • Support additional community-based behavioral health programming as assigned
  • Provide approximately 12 hours per week of in-person behavioral health services within the San Miguel County Jail
  • Conduct mental health and substance use screenings and assessments
  • Complete GAIN assessments and other JBBS-required documentation
  • Conduct suicide risk assessments and crisis intervention services
  • Facilitate discharge planning and community reentry coordination
Requirements
  • Master's degree in Counseling, Social Work, Marriage and Family Therapy, Psychology, or a related behavioral health field
  • Colorado behavioral health license or license eligibility (LPCC, SWC, LMFTC, LPC, LCSW, LMFT, LAC, or equivalent)
  • Experience working with individuals experiencing mental health and/or substance use disorders
  • Strong assessment, treatment planning, and documentation skills
  • Ability to work independently and collaboratively within multidisciplinary teams
  • Ability to provide services in community, home, office, and correctional settings
  • Valid Colorado driver's license, reliable transportation, and proof of automobile insurance
  • Ability to successfully complete CJIS background screening and jail security clearance requirements
