Social Worker, Hospice

Accent Care is seeking a part-time Hospice Social Worker (MSW) to support patients and families facing serious illness. In this role, you will complete psychosocial assessments, develop care plans, and provide counseling to address emotional, social, and practical needs at end of life. You will collaborate closely with nurses, physicians, and chaplains to ensure coordinated, compassionate care. Drawing on Hanford Pharmaceuticals' culture of innovation and collaboration, you'll use evidence-based practices, maintain detailed documentation, and connect families with community resources while upholding the highest quality and safety standards.

Responsibilities

• Provide psychosocial assessments for hospice patients and families
• Develop and implement individualized social work care plans
• Offer counseling and emotional support for end-of-life issues, grief, and adjustment
• Coordinate community resources, benefits, and support services
• Collaborate with interdisciplinary hospice team on care planning
• Educate patients and families on hospice services and advance care planning
• Document assessments, interventions, and outcomes in EHR accurately and timely
• Participate in case conferences and quality improvement initiatives
• Advocate for patient rights and culturally sensitive care
