Senior Community Care, a Program of All-Inclusive Care for the Elderly (PACE), is seeking a compassionate CNA to support seniors in maintaining independence and quality of life. In this on-site role, you will provide hands-on personal care, assist with mobility and activities of daily living, and closely observe and report changes in participants' conditions. Working within an interdisciplinary team, you'll help carry out individualized care plans, support restorative and social activities, and uphold high standards of safety, infection control, and dignity.

Responsibilities

  • Provide direct personal care to elderly participants, including bathing, dressing, grooming, and toileting
  • Assist with safe transfers, mobility, and use of assistive devices
  • Monitor and report changes in participants' condition to nursing staff
  • Support activities of daily living, including meals, hydration, and hygiene
  • Document care provided according to program standards and regulations
  • Assist with restorative and therapeutic activities as directed by nurses or therapists
  • Accompany participants to on-site appointments and activities as needed
  • Follow infection control, safety, and emergency procedures
  • Collaborate with the interdisciplinary care team to support individualized care plans
  • Promote dignity, respect, and emotional support for all participants
