Position Summary
The Physical Therapist (PT) evaluates residents, develops individualized treatment plans, and provides skilled therapy services to improve strength, mobility, balance, safety, and overall functional independence. The PT works closely with physicians, nursing staff, therapy team members, residents, and families to maximize rehabilitation outcomes and quality of life.
Essential Responsibilities
- Perform comprehensive physical therapy evaluations.
- Develop individualized treatment plans based on resident needs, goals, and physician orders.
- Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
- Assess and recommend assistive devices, mobility equipment, and safety modifications.
- Monitor resident progress and adjust treatment plans as appropriate.
- Educate residents, families, and caregivers regarding treatment plans and discharge recommendations.
- Maintain accurate and timely documentation in accordance with regulatory and professional standards.
- Participate in interdisciplinary care planning meetings.
- Ensure compliance with all federal, state, facility, and professional practice requirements.
- Current Colorado Physical Therapist (PT) license in good standing.
- Skilled Nursing Facility, rehabilitation, or long-term care experience preferred.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Commitment to providing exceptional resident-centered care.
- Skilled Nursing Facility (SNF) experience.
- Experience with orthopedic rehabilitation, neurological conditions, balance training, fall prevention, and post-surgical recovery.
- Knowledge of Medicare Part A, Part B, and managed care rehabilitation services.
