$31.20 - $35.35 per hour based on experience; plus $2.75 per hour night shift difference.

Job SummaryThe Licensed Practical Nurse (LPN) will provide nursing care under the direction of a RN in accordance with state and local laws, institutional policies, and professional guidelines. Duties include assessment, monitoring, and documentation of resident status, and collaboration with interdisciplinary teams.
Responsibilities- Nursing Unit Functions
- Model positive communication skills with staff, residents, families, and agencies.
- Listen to shift reports and maintain open communication with supervisors and coworkers.
- Make rounds to observe residents’ physical and psychosocial status.
- Assign personnel to meet resident needs and ensure assignments are carried out.
- Resident Care Functions
- Maintain competence in required skills and treat residents with respect and dignity.
- Observe, record, and report residents’ conditions, reactions to meds, and significant events.
- Participate in resident care planning as assigned by the Director of Nursing.
- Immediately report any suspected abuse or neglect per policy.
- Perform nursing techniques for comfort and well‑being.
- Administer prescribed medications and treatments per providers’ orders.
- Follow shift routines, policies, and procedures for vitals, weight, medications, and treatments.
- May administer services such as catheterization, tube feedings, suctioning, bandaging, colostomy, and drainage bag management.
- Provide range of motion exercises, end‑of‑life care, and discharge as scope permits.
- Report physician/provider calls regarding resident condition changes and document them.
- Update families on resident condition and document communication.
- Follow emergency protocols and advanced directive procedures (DNR/DNI/CPR/Comfort Care).
- Charting and Documentation Functions
- Maintain accurate, timely documentation of medications, treatments, restraints, psychotropic drugs, and skin condition.
- Contribute to completing and updating resident care plans under RN direction.
- Perform routine charting duties per established policies.
- Transcribe physician orders accurately and forward information to other departments.
- Document and report all events involving residents, visitors, and staff.
- Enter nurse notes in a descriptive manner reflecting care provided and resident response.
- Collect ongoing data and complete required forms.
- Use Electronic Health Record efficiently for documentation.
- Personnel Functions
- Attend scheduled nurse meetings.
- Maintain a clean, safe, and efficient work environment.
- Fulfill continuing education requirements.
- Participate in performance evaluations and advise on staffing needs.
- Engage in disciplinary actions as directed by supervisor when warranted.
- Develop and maintain inter‑departmental relationships.
- Report occupational exposures per policy and procedures.
- Attend required in‑services and complete online modules.
- Committee Functions
- Serve on and participate in facility committees to enhance interdisciplinary processes.
- Other duties as assigned.
- Graduate from an accredited nursing school and hold current Colorado LPN license.
- Maintain current CPR certification.
- Effective communicator with staff, families, and external agencies.
- Ability to prioritize duties and perform under pressure.
- Customer‑service oriented with knowledge of successful techniques.
- Understand residents’ charts, doctor’s orders, plan of care, and medication orders.
- Gather clinical data regarding resident status and condition.
- Proficient with computers and electronic devices for timekeeping, in‑service, and documentation.
- Maintain IV certification if required by state practice.
- Understand Antibiotic Stewardship, Infection Prevention/Control, and OSHA guidelines.
