Purpose

The Behavioral Health Registered Nurse (RN) is a highly qualified health care provider who, through education and experience, applies the art and science of nursing to care for patients with behavioral health concerns.

Scope & Objective

Applies specialized psychiatric nursing knowledge in assessment, diagnosis, and treatment of patients with behavioral health concerns. Assists the Lead Behavioral Health RN in building and sustaining the behavioral health program for patients receiving care in homes or assisted living facilities. Functions under the direction of the Nursing Supervisor and the Director of Patient Clinical Services. Initiates case findings, health teachings, counseling, and care that supports life or restores well-being.

Physical

Demands

Position requires frequent talking, hearing, standing, walking, bending, kneeling, stooping, and lifting or moving items weighing up to 50 pounds. Very active throughout the day.

Travel

Requires daily travel within the Front Range of Colorado.

Tasks/Duties &

Job Responsibilities
  • Comply with accepted ethical conduct and Professional Standards of Nursing Practice as set forth by the American Nurses Association.
  • Maintain patient safety and dignity by adhering to Federal, State, Local, and company policy and procedures regarding protected health information and HIPAA.
  • Assess client condition and needs, collect health data, recognize, interpret, and report symptoms and psychological responses, and re-evaluate client nursing needs as required.
  • Develop and implement nursing plan of care, consider client dignity and desires, organize and participate in direct client care, perform treatment, and administer medications.
  • Formulate, notify, and carry out client care plan prescribing achievable goals based on client needs.
  • Maintain a safe, therapeutic environment for the client.
  • Initiate and participate in the development of the Plan of Treatment, obtain physician’s orders, clarify those orders, obtain necessary revisions, inform physician of changes, and provide written summary reports.
  • Participate in health counseling and teaching with the client and family members.
  • Maintain detailed records of nursing actions, client progress, and therapy responses to document and evaluate the Plan of Care.
  • Record clinical/progress notes in the client’s record; supervise care by ancillary personnel; initiate preventive and rehabilitative nursing procedures.
  • Coordinate discharge planning and prepare discharge summaries and instructions.
  • Communicate information effectively to appropriate personnel to facilitate continuity of care.
  • Maintain competency through in-service and continuing education programs.
  • Participate in monthly, quarterly, annual trainings and zone meetings.
  • Coordinate nursing, home health aide, physical therapy, occupational therapy, speech therapy, and other services delivered to clients of the agency.

Compensation

Optimal compensates employees based on a per-visit rate.

Qualifications
  • Minimum of 2 years of experience in Psychiatric Inpatient, Outpatient, or Home Care treatment setting.
  • Currently licensed and in good standing in the state of Colorado.
  • Ability to communicate in the client’s language and document client-related activity appropriately.
  • Adequate written and verbal communication skills.
  • Adequate interpersonal skills.
