Overview

Licensed Practical Nurse (LPN) – Long-Term Care, Springfield, Colorado, United States. Travel assignment lasting 13 weeks, starting 07/06/2026, with guaranteed 36 hours per week. Shift: 3x12-hour days. Assignment is in a Critical Access Hospital setting. This role focuses on delivering direct and indirect resident care, medication administration, and collaboration with a multidisciplinary team to support residents in a long-term care context.

Responsibilities
  • Provide direct and indirect nursing care of residents during the assigned shift
  • Administer medications and treatments as ordered by physicians, and document accordingly
  • Assign, direct, and supervise resident care; oversee CNAs to ensure tasks are completed
  • Communicate resident needs and changes to the Director of Nursing, Assistant Director of Nursing, and/or Resident Care Coordinator; keep physicians and families informed as needed
  • Maintain accurate nursing notes and timely documentation; participate in meetings and required educational sessions
  • Promote teamwork across staff, shifts, and departments; report changes promptly and coordinate with RN as Charge Nurse when applicable
Requirements
  • Active LPN license (Colorado) or eligible for practice in the state
  • Ability to administer medications and treatments per physician orders
  • Experience supervising CNAs and coordinating resident care
  • Strong documentation and charting skills; effective communication with residents, families, and the care team
  • Ability to work 3x12-hour shifts and maintain a 36-hour weekly schedule
  • Comfortable working in a long-term care setting within a Critical Access Hospital
