River Valley Rehabilitation & Healthcare Center is seeking a compassionate and detail-oriented MDS/Care Plan Coordinator to join our growing team! This leadership role is responsible for overseeing the integrity of the facility’s Medicare and Medicaid programs while ensuring residents receive high-quality, individualized care.

Coordinate and manage MDS, PPS, and OBRA assessment schedules
Complete accurate and timely resident assessments and care plans
Monitor Medicare and Medicaid eligibility and coverage requirements
Conduct pre‑admission nursing screenings as assigned
Coordinate interdisciplinary Medicare meetings and care plan conferences
Ensure timely submission of documentation and regulatory reports
Collaborate with nursing staff, therapy teams, physicians, residents, and families
Monitor resident condition changes and update care plans as needed
Ensure compliance with federal, state, and facility regulations
Support quality assurance initiatives and reimbursement accuracy
Current Colorado LPN or RN license required
Graduate of an accredited nursing program
Minimum of 2 years of long‑term care experience preferred
Knowledge of RAI, Medicare, Medicaid, and PPS processes required
Strong communication, organization, and leadership skills
Detail‑oriented, dependable, and team‑focused professional
Passionate about providing resident‑centered care and clinical excellence
