Position Overview

As an LPN at the Brush Family Practice Clinic, we are offering a full-time LPN position. The role involves assisting clinicians and nursing staff in providing medical care, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment, and skills to provide an excellent patient experience. The position offers shifts at both East Morgan County Hospital Clinic and our Brush Clinic.

The schedule is Monday–Friday with Saturday rotations.

Position Summary

This position provides nursing care under the direction and supervision of a registered nurse and/or licensed physician, and is accountable for the quality of nursing services delivered by self or others who are under their direction. Responsibilities include observing, contributing to the plan of care, implementing, reporting and documenting patient care in accordance with department policies, standard of care and state scope of practice.

The position utilizes specialized knowledge and judgment to assess data and plan, provide, and evaluate care appropriate to the physical and developmental age of assigned patients.

Core Functions
  • Observe, compile, and report the patient’s physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Review available information obtained by health care team members.
  • Contribute to plan of care under the direction of the registered nurse, including the discharge plan, utilizing assessment data and patient, family and health team input. Plan care in collaboration with multidisciplinary team members. Review plan of care to reflect changing patient needs and provide input for updates as needed.
  • Implement care based on delegated interventions identified in care plan and medical orders, within scope of practice. Provide care in compliance with policies, procedures, standards of care and regulatory requirements. May delegate and supervise non‑licensed personnel per state scope of practice. Communicate accurately and completely to other caregivers.
  • Report patient condition, including signs or symptoms indicating change, to the registered nurse and/or physician. Observe and report patient progress toward goals and expected outcomes. Observe and report patient’s response and effectiveness of teaching.
  • Document observation, implementation and data in the patient record. Reflection of objective/subjective data, interventions, and response to treatment. Note physician orders accurately and timely.
  • Plan, organize and coordinate completion of documentation and input of patient assessment data as required by federal regulation. Maintain, update, and revise documentation per federal and state guidelines. Communicate and coordinate with interdisciplinary team regarding functional/care level and transitions. Perform audits related to reimbursement documentation and quality management. Review audit results and present summary information to management.
  • Coordinate admission process including new resident evaluations. Provide verbal and written communication to residents, families and referral sources about the admission process. Facilitate problem solving with residents, families, referral sources, insurance companies and clinical staff.
  • Work under supervision, prioritize data from multiple sources to provide quality care and support to the patient and family. Incumbents work in a fast‑paced, sometimes stressful environment with a strong focus on customer service. Interact with staff at all levels, patients, families and physicians.
  • Minimum Qualifications

    Must possess a current, valid LPN or LVN license in the state of practice, or a temporary/compact LPN/LVN license. BLS certification is required. Additional certification or continuing education may be required based on area of practice. Completion of a state‑approved IV therapy program may be required in certain facilities or departments.

    Banner Registry and Travel positions require a minimum of one year of experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as an LPN or LVN in the specialty area. Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain it during employment.

    Preferred Qualifications

    Experience in a designated clinical area.

    Additional related education and/or experience.

    Estimated Pay Range

    $25.54 – $38.30 per hour
