Position: Travel Licensed Practical Nurse Long-Term Care Job
Location: Gunnison

Overview

Licensed Practical Nurse, Long-Term Care (Memory Care) – Start August 3, 2026; 13-week travel temporary assignment;
Weekly pay between $1,283 and $1,303 per week;
Guaranteed hours 36 per week;
Shift: Days, 12‑hour shifts (6:00 a.m.–6:00 p.m.), 3 shifts per week;
Work setting: memory care within a long-term care environment. This position is with Healthforce Healthcare LLC and is intended for an experienced LPN to support rehabilitative and restorative nursing across a memory care resident population.

Responsibilities
  • Provide direct nursing care to residents in a memory care setting under the direction of the nursing leadership.
  • Administer medications and treatments per physician orders; monitor for adverse reactions and document outcomes.
  • Assess residents, update care plans, and communicate changes to the health care team; participate in care conferences as needed.
  • Supervise and support day‑to‑day nursing activities; oversee nursing assistants during the shift to maintain safe, high‑quality care.
  • Collaborate with the interdisciplinary team to implement rehabilitative and restorative nursing interventions and document progress.
  • Ensure compliance with applicable Federal, State, and local regulations, standards, and guidance governing long‑term care and memory care services.
Requirements
  • Active LPN license in Colorado.
  • Minimum 3 years’ experience in a hospital, skilled nursing facility, or comparable setting; training in rehabilitative and restorative nursing practices.
  • IV certification preferred for LPNs; current BLS certification.
  • Ability to work in a memory care long‑term care environment with residents who may have dementia or related conditions.
  • CAPS background check required.
  • Reliable transportation required due to rural setting.
