As a CNA, you’ll play a vital role in supporting our patients’ quality of life through consistent, one-on-one, relationship-based care in their homes. Compensation$16.00–$22.00/hr, depending on experience.
$1/hr shift differential for nights and weekends.

Key Responsibilities- Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
- Provide companionship, emotional support, and social engagement.
- Support meal preparation, nutrition support, and light housekeeping.
- Monitor and report changes in condition.
- Follow care plans and document accurately using EMR standards.
- High school diploma or GED.
- Active CNA certification in Colorado.
- CPR certified.
- Valid driver’s license and reliable transportation.
- Ability to work independently in a non-facility setting.
- Home health, long-term care, or hospice experience preferred.
