As a CNA, you’ll play a vital role in supporting our patients’ quality of life through consistent, one-on-one, relationship-based care in their homes. $16.00–$22.00/hr, depending on experience.

$1/hr shift differential for nights and weekends.

PRN

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
• Provide companionship, emotional support, and social engagement.
• Support meal preparation, nutrition support, and light housekeeping.
• Monitor and report changes in condition.
• Follow care plans and document accurately using EMR standards.

• High school diploma or GED.
• Active CNA certification in Colorado.
• CPR certified.
• Valid driver’s license and reliable transportation.
• Ability to work independently in a non-facility setting.
• Home health, long-term care, or hospice experience preferred.
