Position SummaryThe Registered Nurse (RN) provides skilled nursing services to patients in their homes in accordance with physician orders and individualized care plans. This position works collaboratively with physicians, caregivers, and the interdisciplinary team to ensure high-quality patient outcomes.
Service Areas- Broomfield and surrounding communities
- Provide skilled nursing care according to physician orders and patient care plans.
- Conduct patient assessments and ongoing evaluations.
- Coordinate care and communicate patient status changes with physicians and care team members.
- Educate patients and families on disease management, medications, and home care needs.
- Complete accurate and timely clinical documentation, including OASIS assessments.
- Participate in care planning, case conferences, and quality improvement activities.
- Supervise LPNs, CNAs, and Home Health Aides in accordance with regulatory requirements.
- Support patients' preventive, rehabilitative, and ongoing healthcare needs.
- Active Registered Nurse (RN) license.
- Graduate of an accredited nursing program.
- Current CPR certification.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently and manage time effectively.
- Minimum one (1) year of nursing experience.
- Home health experience preferred.
