Position Summary:

The Nurse Practitioner will provide safe, effective, and quality primary, preventive, and evidence-based health care to patients in a culturally competent manner, focusing on health promotion and disease prevention while upholding the ASQ (Access, Stewardship, and Quality) pillars.

The Nurse Practitioner must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient s needs. The Nurse Practitioner will provide patients, families, and personnel with a safe and confidential environment.

Essential Functions:

· Interview patients to obtain accurate, thorough health histories.

· Performs physical examinations and provides preventive health services.

· Orders, interprets and evaluates diagnostic tests to identify and assess patients  clinical problems and health care needs.

· Formulates comprehensive health care plan based on findings.

· Prescribes or recommends medications or other forms of treatment.

· Refers and consults with other professionals as needed.

· Educates patients and family members about health care issues.

· Ensures appropriate patient follow-up and reevaluation occur.

· Actively participates in the integrated health care team, working conjunctively with the School Based Health Clinic, Care Team, and dental and behavioral health departments to ensure seamless, multidisciplinary patient care.
