Horizons Care Center seeks a compassionate Bath Aid/CNA to provide dignified bathing, personal hygiene, and basic nursing care to residents in our skilled nursing and rehab community. In this role, you'll assist with showers, bed baths, grooming, toileting, and mobility while closely monitoring comfort, safety, and skin integrity. You'll collaborate with nurses and therapists, follow individualized care plans, document care accurately, and support a resident centered environment built on respect, teamwork, quality, and open communication.
This position offers the opportunity to build strong relationships and make a daily impact on residents' quality of life.
Responsibilities
- Provide safe, dignified baths, showers, and bed baths per resident preference and care plans
- Assist with grooming, toileting, dressing, and personal hygiene to support comfort and dignity
- Support safe transfers, repositioning, and mobility using proper body mechanics and equipment
- Observe and report changes in residents' skin condition, comfort, or health status to nursing staff
- Maintain infection control, safety, and fall prevention practices at all times
- Accurately document care provided in electronic or paper records as required
- Collaborate with nurses, therapists, and other team members to deliver coordinated, resident centered care
- Communicate respectfully with residents and families, supporting emotional comfort and trust
- Keep bathing areas, equipment, and supplies clean, stocked, and in good working order
- Participate in training, huddles, and quality initiatives that promote safety, teamwork, and high quality care
