- CORRECTIONS CASE MANAGER II

Responsibilities
  • Act as a work leader, overseeing the work product of assigned Case Manager I staff, directing, training, mentoring, and coaching. Provide backup and support as needed.
  • Create and maintain an interdependent approach with internal and external stakeholders to guide offenders in accordance with CDOC needs, regulations, federal laws, state statutes and case‑management goals, ensuring continuity of care from intake to release:
    • Admission, assessment and re‑entry planning
    • Implementation, progress assessment and modifications
    • Parole planning and referral to community resources
  • Utilize supportive inquiry and solution‑focused dialog to motivate offenders to be accountable for their facility programming and transitional needs.
  • Assist inmates with linking to programs and services within the facilities that address critical needs such as education, medical or mental health issues, substance abuse, cognitive‑behavioral treatment, and offense‑specific interventions.
  • Oversee delivery of case‑management services as designed in policy and training, targeting criminogenic needs through risk/needs assessments, case planning, and cognitive interventions.
  • Ensure that Case Managers I use motivational interviewing and cognitive‑based interventions, and that case planning aligns with the Colorado Transitional Accountability Plan (CTAP).
  • Oversee management of inmate cases from intake to pre‑release planning, ensuring court‑ordered programs and conditions are met prior to release.
  • Maintain successful case‑management operations through tracking service delivery to offenders and monitoring:
    • Collection and evaluation of information to determine custody levels and programming needs
    • Development of individual case planning through completion of assessments
    • Documentation of inmate behaviors
  • Provide individual guidance to Case Managers I to assist them with inmate‑based service delivery.
  • Utilize a multidisciplinary and collaborative approach with internal and external stakeholders to guide offenders in accordance with CDOC policy, ensuring continuity of care and compliance with state statute and CDOC administrative regulations.
  • Educate, negotiate, and defend case‑management services and activities with internal partner programs, external agencies and the community.
  • Coordinate programming based on offender needs, determine earned time grants, develop parole plans, develop community corrections referrals, and act as the primary contact and source of information for facility administration, central administration, the Parole Board, outside agencies, attorneys, families, victims, and offenders.
  • Perform other job‑related duties as assigned or required.
Minimum Qualifications
  • Four (4) years of corrections experience, including at least one (1) year providing case‑management services to incarcerated individuals (case planning, custody level determination, earned time grants, parole planning/references, and linking inmates to programs).
