Older Adults Coach
Manna Resource Center is a faith-based nonprofit organization founded to strengthen individuals and families in Douglas County, Colorado and the surrounding area through programs that provide holistic care and support in the areas of self-sufficiency, wellness, and family development. Manna Resource Center operates three core programs:
Manna Resource Center Coaching, Prevention Family Support, and Manna Community Wellness. The Manna Resource Center Coaching and Prevention Family Support programs integrate coaching, community engagement, resource navigation, and family support to help participants and their families build greater self-sufficiency. Manna Community Wellness complements these services by providing professional mental health support to Manna clients.
The Older Adults Coach provides comprehensive, voluntary services to adults age 60 and older who are experiencing poverty, financial insecurity, or other barriers to stability. The Coach works with older adults to identify strengths and needs, navigate benefits and community resources, complete applications, and establish goals that promote greater stability and self-sufficiency. Services are individualized and may include intensive resource navigation, specialized case management, education, and coaching.
Major areas of responsibility include:
- Conduct an initial assessment using Part A of the Colorado Family Support Assessment 2.0 (CFSA2) to identify strengths, needs, vulnerabilities, and areas of crisis.
- Work collaboratively with participants to identify goals and determine the appropriate level of support needed.
- Develop individualized plans with measurable goals related to stability and self-sufficiency.
- Provide ongoing coaching and support based on participant needs.
- Support older adults in strengthening stability in areas such as housing, transportation, healthcare, food security, and debt management.
- Provide individualized resource navigation and benefits assistance.
- Educate participants about available benefit programs, including Medicare, SNAP, Social Security, and other applicable resources.
- Assist participants with completing and submitting benefit applications.
- Connect participants to community resources related to transportation, housing, healthcare, financial assistance, food security, and other identified needs.
- Help participants understand and navigate complex systems while reducing confusion, frustration, and barriers to accessing services.
- Provide older adult education and information related to available benefits and community resources.
- Maintain accurate and timely participant records, case notes, goals, services, and outcomes in empowOR.
- Track services provided, measurable goals, participant progress, benefit enrollment, and outcomes.
- Document participant engagement and progress in accordance with Manna Resource Center and grant requirements.
- Develop and maintain collaborative relationships with community organizations and service providers.
- Collaborate with the Douglas County Department of Human Services eligibility and adult protection teams to assist older adults with benefits and other significant needs.
- Participate in training provided by the Douglas County Department of Human Services related to benefit programs, application processes, and adult protection.
- Coordinate with Aging Resources of Douglas County as a referral source for older adults.
Program operations responsibilities include:
- Manage a caseload of older adult participants based on individual needs and level of support required.
- Respond to referrals and engage older adults in appropriate services.
- Provide services through a variety of settings, including the Manna Resource Center office, virtually, in the community, and in participants' homes.
- Monitor participant outcomes and contribute to program evaluation and continuous improvement.
- Participate in staff meetings, trainings, and other activities that support effective service delivery.
- Follow all Manna Resource Center policies, procedures, confidentiality requirements, and grant-related expectations.
Bachelor's degree, preferably in a human service related field. A deep level of knowledge and understanding of the complexity of poverty and familiarity working with issues of poverty. Strong ability to build trusting, respectful relationships with older adults and individuals experiencing financial or other significant barriers. 3+ years experience/certification in coaching, family development, or social work is a plus. Strong interpersonal, written, and oral communication skills;
