Overview
Paid one-on-one training and education provided. Empower, engage and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment. Primarily works in the clinic setting and conducts initial home visits with another staff member present. Acts as an onsite Patient Educator and Case Manager for patients with various shifts and work hours. Functions as part of the Home Therapies dialysis health care team to ensure quality home patient care in accordance with company policies, procedures and training.
Supports the Quality Enhancement Program (QEP) and CQI activities related to patient satisfaction and process improvement to achieve QEP goals.
- Staff Related: Provide safe and effective training and oversight of home dialysis therapies to patients in compliance with facility policies, procedures, and regulatory standards. Rotate with other licensed staff to ensure coverage. Participate in staff training, orientation of new staff, and required staff meetings.
- Quality: Deliver high-quality patient care aligned with company quality goals and standards. Submit CQI information for HT QAI template and participate in monthly CQI meetings.
- Education: Identify and evaluate candidates for the Home Therapies Program, assess home environments, and train patients and families on safe operation and maintenance of home therapies equipment through a formal program. Educate patients on ESRD and the Kidney Options Program. Assess readiness and suitability of home environments for ongoing home dialysis therapy. Provide ongoing education about renal dialysis, peritoneal or vascular access, and related health conditions.
- Dialysis Treatment: Provide safe and effective home therapy support. Assess patient response to treatment using predefined protocols. Communicate patient-related issues to the physician as needed. Report adverse events and equipment/technology problems with proper documentation. Coordinate transfer of patients to the hemodialysis clinic when appropriate.
- Laboratory: Review, transcribe, and enter physician orders into the Medical Information System. Follow policies and regulations for collection, packaging, and reporting of lab specimens.
- General Duties: Maintain competency with emergency procedures, initiate CPR as needed, administer medications per algorithms, and document appropriate medical justification. Maintain records on controlled substances as required by law.
- Maintenance/Technical: Maintain skill level with home dialysis equipment, troubleshoot with patients by phone, guide preventive maintenance, and coordinate equipment delivery/maintenance with the products division.
- Medical Records & Documentation: Admit new patients, document orders, labs, vital signs, treatment parameters, patient status, and ensure proper entry into the system. Ensure adherence to admission, transfer, and discharge policies. Complete monthly and annual reporting as required.
- Other: Perform other related duties as assigned.
The role involves desk work, computer use, and interaction with patients and staff. Travel between facilities and various community locations is required. Some meetings may require travel to regional, business unit, or corporate sites. Work environment is healthcare with controlled temperature and moderate noise. May be exposed to infectious materials. Ishihara color vision testing is required as a condition of employment;

Education & Required Credentials- Graduate of an accredited School of Nursing (R.N.).
- Current appropriate state licensure.
- Minimum of one year medical-surgical nursing experience preferred; ICU experience preferred.
- Minimum of one year dialysis experience preferred; peritoneal experience preferred.
- Successfully complete a training course in the theory and practice of dialysis therapies.
- Competent with operating all Home Therapies equipment.
- CPR certification with maintenance as required.
- Icd-9 coding training.
- Demonstrated teaching abilities.

Compensation & BenefitsHourly Rate: $35.58 - $59.28. Non-Bonus Eligible Positions.
