Role Description

The Nurse Navigator is a full-time, on-site role located in Columbia City, IN, focused on guiding patients through the healthcare system and coordinating care across services. Day-to-day responsibilities include assessing patient needs, providing education on diagnoses and treatment plans, and serving as a primary point of contact for patients and families. The Nurse Navigator collaborates with physicians, nurses, and other care team members to schedule appointments, facilitate referrals, and ensure timely follow-up and continuity of care.

This role also involves monitoring patient progress, addressing barriers to care such as transportation or insurance issues, documenting interactions in the medical record, and participating in quality and safety initiatives to support optimal outcomes.

Qualifications
  • Clinical expertise in nursing practice, including patient assessment, care coordination, and health education.
  • Strong communication and interpersonal skills to effectively support patients, families, and interdisciplinary teams.
  • Organizational and time-management abilities to handle multiple patients, priorities, and documentation requirements.
  • Proficiency with electronic health records and basic computer skills for scheduling, charting, and data tracking.
  • Active Registered Nurse (RN) license in the state of Indiana.
  • Preferred experience in care navigation, case management, or a related patient support role.
  • Knowledge of healthcare systems, community resources, and insurance processes is beneficial.
  • Commitment to patient-centered care, safety, and continuous quality improvement.
