Responsibilities: Assess the physical abilities and needs of patients in a skilled nursing facility, developing individualized therapy plans. Provide physical therapy treatments to improve mobility, strength, and overall functional ability.
Assist patients with recovery from illness, injury, or surgery, aiming to restore independence and quality of life. Collaborate with an interdisciplinary team, including physicians, nurses, and occupational therapists, to coordinate care. Educate patients and their families on therapeutic exercises, home programs, and strategies to maintain progress post-discharge. Maintain accurate and timely documentation of patient assessments, treatments, and progress. Regularly reevaluate patients and adjust treatment plans as necessary to achieve optimal outcomes.
Requirements: Current Physical Therapist (PT) license in good standing. Minimum of 2 years of experience in a skilled nursing facility (SNF) or similar setting. Strong knowledge of rehabilitation techniques, therapy equipment, and treatment planning in a long-term care environment. Excellent communication and interpersonal skills. Ability to work collaboratively with a multidisciplinary team and independently. Compassionate, patient-centered care with a focus on enhancing patients' quality of life.
