Mercy South Hospice is seeking an RN Intake for the St. Louis area to manage referrals for Hospice, Home Health, and Home Infusion across acute care, clinics, and select nursing homes.
You will coordinate with discharge planners, social workers, and physicians to ensure timely discharges and continuity of care within the Mercy system. The role requires an RN with 3+ years of nursing experience and prefer Hospice/Home Health background; CHPN certification is a plus.
