JOB SUMMARYThe Medical Social Worker (LCSW-C) manages the patient’s psychosocial care from admission through discharge. Works collaboratively with patients, families, the interdisciplinary team and external providers to ensure safe, timely, cost-effective care. Develops and oversees individualized care plans, serves as a problem‑solver and resource, and drives continuous improvement in patient outcomes. Strong internal communication skills are required to gain cooperation and implement change.
Key Responsibilities- Provide age‑appropriate assessment and interventions across assigned populations (infant, child, adolescent, adult, geriatric).
- Interpret assessment data to identify patient needs and tailor the plan of care.
- Use developmentally appropriate communication and services consistent with unit scope of practice.
- Maintain active presence on units and collaborate with team members.
- Complete assessments for new admissions within 24 hours.
- Graduate of an accredited college/university.
- Master’s degree in Social Work required.
- Acute care medical experience preferred.
- LCSW‑C licensure required.
Minimum 34.62/hour - Maximum 57.19/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
