Reporting to the Transitional Age Youth Transition Coordinator, the Transitional Age Youth (TAY) Coordinator is responsible for providing case management services in collaboration with the Transition Coordinator and other cross-functional teams.
The TAY Case Coordinator will focus on building a partnership with young persons to create a solid foundation for the transition into adulthood through intensive case management services; housing stabilization; job coaching and access to supported employment opportunities; resource procurement and advocacy. The TAY Case Coordinator is responsible for engaging participants in immediate needs assessment and procurement of resources as a part of a service delivery team.
The positions work within the limitations of the organization’s resources and in compliance with grants, contracts, and agency policies and procedures.

- Bachelor’s Degree in human services or another related field.
- Two years of relevant experience with transitional-age youth.
- Valid driver’s license and access to reliable transportation.
- At least 2 years of driving experience required.
- Ability to relate well to staff and board members of different races, ethnicities, genders, sexual orientations, socioeconomic backgrounds, and religions.

To that end, every employee will promptly refer any consumer with an interest in obtaining supported employment to appropriate Vocational Staff and will actively participate as needed in the Evidence-Based Practice in supported employment initiative/model.
