Job Summary
The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during a patient’s hospital stay and coordinates planning with other disciplines utilizing a patient/customer driven approach.
Responsibilities- Perform comprehensive nursing assessments and reassessments.
- Conduct age‑appropriate admission or transfer assessments, obtaining input from families or guardians when appropriate.
- Document findings accurately and completely.
- Assess post‑operative and post‑invasive procedure patients.
- Assess and document education and discharge needs of patients and families on admission and throughout hospitalization.
- Reassess patients and document pertinent observations according to the patient plan of care, changes in condition, status or diagnosis, response to care, procedures, and standards of care.
- Establish, coordinate, and evaluate a plan of care based on assessment data, patient diagnosis, lab results, tests, physician orders, protocols, standards of care, and other relevant information.
- Identify short‑term and long‑term goals based on patient care needs.
- Formulate nursing interventions to achieve desired patient outcomes.
- Incorporate disease‑specific evidence‑based practice into the nursing care plan and documentation.
- Provide and document nursing interventions based on assessed patient needs, the plan of care, and changes in patient status.
- Collaborate with health team members to coordinate the daily plan of care for assigned patients.
- Provide, coordinate, and communicate patient care, including accurate handoff communication reports.
- Administer and document medications accurately according to policies and procedures.
- Monitor, maintain, and document accurate IV fluids and parenteral nutrition according to policies and procedures.
- Complete referrals as indicated by assessment data.
- Request consultation for specialized needs, equipment, or patient/family information.
- Provide patient/family education and discharge planning according to documentation guidelines and protocols.
- Clarify physician orders as warranted; acknowledge and implement orders accurately and in a timely manner.
- Assist physicians with procedures or treatments as requested or delegate to a care partner as appropriate.
- Document “Readback” for all telephone or verbal orders; take verbally ordered instructions only in emergency situations.
- Recognize changes in patient condition and take appropriate nursing actions.
- Involve family or guardian in care and decision‑making when appropriate.
- Identify risks to patients and take appropriate actions.
- Incorporate infection control practices into daily care.
- Document or communicate changes in nursing care or patient condition.
- Perform ongoing evaluation of care effectiveness based on assessment data, nursing interventions, patient responses to medications, treatments, and procedures.
- Evaluate the effectiveness of patient and family education.
- Modify the plan of care as indicated.
- Recognize significant changes in clinical parameters and report immediately to the physician and others as needed.
- Identify problems, gather relevant data, suggest solutions, communicate with appropriate lines of authority, and work toward resolution.
- Report variations from care or treatment following reporting policies and procedures.
- Provide specialized care to patients at high risk for injury, including restraint care, pain management, and abuse assessment.
- Initiate, evaluate alternatives to restraints, apply restraints per approved procedures, monitor patient response, provide specified care, and document use thoroughly.
- Assess for pain, incorporate cultural and spiritual beliefs into pain management, implement techniques, and involve patients and families in pain management planning.
- Recognize and respond to signs of abuse or neglect, reporting to the appropriate authorities and taking necessary actions to support patient safety.
- Adhere to all quality and performance standards, policies, procedures, and protocols for clinical and technical aspects of care.
