Clinical Nurse Coordinator BMT & Cell Therapy

Additional Information About the Role

Inpatient Nurse Coordinator within the Hematologic Malignancy Program (formerly BMT). The position will support 10800 which is one of the HMP floors dedicated to Lymphoma and Multiple Myeloma patients who are admitted for new diagnosis work-up, transplant or cellular therapy, planned chemotherapy regimens or complications from transplant, CAR-t or chemo.

Onsite Work at Barnes Jewish Hospital.

BSN Candidate is ideal. Care coordination and/or oncology experience preferred.

Coordinates the health care of select patient populations/designated service across the continuum of care. Responsible for coordination of patient contact, patient education, prepares patient/family for procedure/test in collaboration with the healthcare team.

• Facilitates patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
• Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice.
• Monitors care and data to optimize patient outcomes.
• Facilitates education of patients and families. Acts as a resource to the health care team.
• Ensures actions meet and support the overall goals of the business.
