Mercy is seeking an RN Care Manager to join our Healthcare & Medical Services department. In this role, you will coordinate and manage patient care plans, collaborate with physicians and interdisciplinary teams, and support patients and families through transitions of care. You will assess patient needs, monitor progress, and advocate for quality, cost-effective outcomes. Responsibilities- Perform comprehensive nursing and psychosocial assessments for weekend admissions and existing patients requiring care management support.
- Develop, implement, and update individualized care plans that address medical, functional, and social needs.
- Coordinate safe and timely discharge planning, including home health, rehabilitation, equipment, medications, and follow-up appointments.
- Collaborate closely with physicians, bedside nurses, social workers, therapists, and other interdisciplinary team members to guide care progression.
- Educate patients and families on diagnoses, medications, self-care strategies, and discharge instructions to promote understanding and adherence.
- Identify and address barriers to care, including social determinants of health, safety concerns, and resource limitations.
- Conduct utilization review and help ensure appropriate level of care and efficient use of hospital and community resources.
- Maintain accurate, timely, and thorough documentation in the electronic health record for all care management activities.
- Prioritize and manage a weekend caseload, responding promptly to consults and referrals and adjusting plans based on patient acuity.
- Participate in quality improvement initiatives and use evidence-based practices to enhance patient outcomes and reduce readmissions.

- Bachelor of Science in Nursing (BSN) preferred or equivalent experience
- Basic Life Support (BLS) certification required
- Advanced Cardiovascular Life Support (ACLS) preferred, depending on unit
- Case Management certification (CCM, ACM-RN, or similar) preferred
- Experience with electronic health records (EHR) systems
- Prior inpatient, acute care, or case management experience
