Signify Health seeks a Full Time Clinician (Nurse Practitioner) to perform in-home health evaluations across a defined coverage area, traveling up to 100% and possibly overnight. The role uses an iPad-based clinical workflow and may include additional diagnostic tests as required by health plans.
The ideal candidate holds an active, unrestricted license in the coverage area, is board-certified, and can obtain multi-state licenses. Travel to counties within state and beyond is expected.
